| ○ | ○ | Article | Spinal Stenosis Grading in Magnetic Resonance Imaging Using Deep Convolutional Neural Networks | Study Design. Retrospective magnetic resonance imaging grading with comparison between experts and deep convolutional neural networks (CNNs). Objective. This study aims to verify the feasibility of a computer-assisted spine stenosis grading system by comparing the diagnostic agreement between two experts and the agreement between the experts and trained artificial CNN classifiers. Summary of Background Data. Spinal stenosis grading is important; however, it is tedious job to check the MR images slide by slide to classify patient grades often having different opinions regarding the final diagnosis. Methods. For 542 L4-5 axial MR images, two experts independently localized the center position of the spine canal and graded the status. Two CNN classifiers each trained with the grading label made by the two experts were validated using 10-fold cross-validation. Each classifier consisted of a CNN detection model responsible for the localization of patches near the canal and a classification CNN model to predict the spinal stenosis status in the localized patches. Faster R-CNN was used for the detection model whereas VGG network was used for the classification model. A comparison in grading agreement was carried out between the two experts as well as that of the experts and the prediction results generated by the CNN models. Results. Grading agreement between the experts was 77.5% and 75% in terms of accuracy and F1 scores. The agreement between the first expert and the model trained with the labels of the first expert was 83% and 75.4%, respectively. The agreement between the second expert and the model trained with the labels of the second expert was 77.9% and 74.9%. The differences between the two experts were significant, whereas the differences between each expert and the trained models were not significant. Conclusion. | ○ | ○ | Article | The Effect of an Educational and Interactive Informed Consent Process on Patients With Cervical Spondylotic Myelopathy Caused by Ossification of the Posterior Longitudinal Ligament | Study Design. Prospective observational cohort study. Objective. In this study, an educational and interactive informed consent (EIC) program was proposed for patients with cervical spondylotic myelopathy caused by ossification of the posterior longitudinal ligament (OPLL-CSM) to improve their comprehension level during the informed consent process. Summary of Background Data. OPLL-CSM is a slow progressive disease, and it is difficult for patients to understand the disease. Few studies have evaluated very specific programs to improve the informed consent process for these patients. Methods. This prospective study evaluated patients with OPLL-CSM who either underwent the proposed EIC process (n = 63) or the standard consent process (n = 124). The standard consent process only included a physician-patient interview. During the EIC process, information was provided regarding OPLL-CSM through information booklets, a video, verbal information, and initial and second physician-patient interviews. After the second physician-patient interview, the patient was requested to answer 14 medical questions to assess their knowledge about OPLL-CSM. The proposed EIC process took approximately 90 minutes. They were asked to report the most useful educational method and the most effective method of reinforcing verbal communication. Results. The mean questionnaire scores were higher in the EIC group than in the control group (P < 0.001). Video was selected by 50/63 patients (79.4%) as the most useful EIC process method, and the most effective method of reinforcing verbal communication was video (n = 61; 96.8%). Patients in the EIC group reported having higher satisfaction with surgery (P = 0.024) than did those in the control group. Conclusion. The proposed EIC process was shown to result in good patient comprehension and recall regarding OPLL-CSM. Using a video was the most informative and effective reinforcement of verbal communication. The enhanced educational group had better knowledge and improved satisfaction following surgery. | ○ | ○ | Article | Treatment Pattern and Economic Burden of Refractory Gastroesophageal Reflux Disease Patients in Korea | Background/Aims The prevalence of gastroesophageal reflux disease (GERD) has increased in Korea, and the economic burden of this chronic disease is substantial. We aim to investigate the treatment pattern and economic burden in Korea of refractory GERD requiring long-term use of proton pump inhibitors (PPIs). Methods We conducted a cross-sectional analysis of nationally representative sample data obtained from 2012 to 2016 and provided by the Health Insurance Review and Assessment Service. The participants were 86 936 in the medication group and 40 in the surgery group. Age- and sex-standardized prevalence of refractory GERD requiring PPIs medication for more than 12 weeks were analyzed. Generalized linear models were used to estimate cost ratios for comparing the medical costs of the surgery and medication groups after adjustment for demographics and comorbidity. Results The prevalence of refractory GERD increased from 1.2% in 2012 to 1.9% in 2016, and the estimated total number of GERD patients increased from 402 000 to 736 000 during this time period. The medical expenditures of the surgery group within 90 days of antireflux surgery (ARS) were 16.9-fold higher compared to those of the medication group; the significant difference in medical costs between the 2 groups disappeared after 90 days post-surgery. Conclusions The prevalence of refractory GERD requiring long-term use of PPIs has been trending upwards recently. Nevertheless, ARS is very rarely performed. | ○ | ○ | Article | Comparison of Microphysical Characteristics between the Southern Korean Peninsula and Oklahoma Using Two-Dimensional Video Disdrometer Data | Differences in atmospheric environments can have a significant impact on microphysical processes of precipitation. Dominant warm (cold) rain processes in East Asia (southern Great Plains of the United States) are implied by a large (small or constant) gradient of reflectivity at low levels in vertical reflectivity profiles. Long-term ground observations using two-dimensional video disdrometers were conducted in the southern Korean Peninsula (KOR) and Norman, Oklahoma, United States (OKL). Raindrop size distributions (RSD) and their moments in the two regions were analyzed in the framework of scaling normalized RSDs. Results show that the concentrations of small (big) raindrops were higher (smaller) in KOR than in OKL. KOR RSDs were also found to be characterized by relatively high characteristic number concentrations N0' and small characteristic diameters D-m' when compared to OKL RSDs. The N-0' increases with increasing D-m' in both KOR and OKL at lower Z with the opposite trend at higher Z. In addition, OKL RSDs with D-m' > .2.5 mm indicate the existence of an equilibrium between coalescence and breakup processes. Rainfall estimation relationships between the rain rate R and radar variables were retrieved from scattering simulations at S-, C-, and X-band wavelengths. KOR RSDs showed relatively small horizontal reflectivity and specific differential phase shift at the same R and same wavelength when compared to OKL RSDs. The regional dependency was significant due to the different microphysical process in KOR and OKL. | ○ | ○ | Article | Irradiation-Induced Intestinal Damage Is Recovered by the Indigenous Gut BacteriaLactobacillus acidophilus | The intestinal tract is one of the most sensitive organs following irradiation. The protective effect of specific indigenous microbiota on irradiation-induced damage to intestinal epithelial cells has not been reported. Mice were irradiated with a single dose of 6 Gy of gamma rays. The intestinal damage was analyzed by histopathology. Intestinal stemness and differentiation were determined by intestinal organoid culture. Microbiota community was observed by high-throughput 16S rRNA gene sequencing and oligotyping analysis. We showed that distal small intestine was damaged by sublethal dose of gamma irradiation. Intestinal organoids derived from the irradiated mice showed defects in budding and mucin expression, suggesting the detrimental effect of irradiation on the intestinal stemness and differentiation. In addition, irradiation reduced intestinal immunoglobulin A level, concomitant with decreased microbiota diversity based on our high-throughput 16S rRNA gene sequencing data. Especially, the relative abundance ofLactobacilluswas reduced at early time point post-irradiation; however, it was recovered at late time point. Oligotyping analysis within theLactobacillusgenus indicated thatLactobacillus-related oligotype 1 (OT1) includingLactobacillus acidophilusmight drive recovery after irradiation as it was associated with increased long-term numbers post-exposure. We showed that treatment with heat-killedL. acidophilusrescued the budding-impaired organoids and induced sufficient differentiation in epithelial cells, and particularly mucin-producing cells, in intestinal organoids. | ○ | ○ | Article | Thermomechanical Actuator Based on a Molecular-Spring Polymer with a Long Alkyl Side Chain | A disubstituted acetylene polymer, with a molecular-spring main chain and a long alkyl side chain of the octadecyl group (PDMC18SP), was examined as an active material for a thermomechanical actuator and its thermodynamic and (thermo)mechanical properties were compared to those of other two polymer derivatives [i.e., a polymer without a long alkyl side chain (PTMSP) and a polymer with the same long alkyl side chain with a n cooling phenyl ring (PDMC(18)SDPA)]. Unlike PTMSP, PDMC18SP and PDMC(18)SDPA produced soft films, which could be considerably elongated by an external tensile force at room temperature. When the external force was removed, PDMC18SP recovered to its original form, while PDMC(18)SDPA did not. Both polymers showed apparent thermodynamic phase transition. PDMC18SP underwent a much greater change under both strain and stress with temperature compared to PDMC(18)SDPA. PDMC18SP was used as an active layer to prepare a bilayer-type actuator, which showed apparent mechanical actuation with temperature. | ○ | ○ | Article | Phenanthrenes isolated from diocorea batatas Decne peel with anti-platelet aggregation activity via direct factor Xa inhibitory activity | Phenanthrene is a class of phenolic metabolites present in Dioscorea batatas which has bioactivities such as antifungal, anti-inflammatory and antioxidative activities. Two phenanthrenes isolated from D. batatas Decne peel, exhibited inhibition against factor Xa and platelet aggregation. They were evaluated for their anti-FXa and anti-platelet aggregation properties by monitoring clotting time, platelet aggregation, FXa activity, and thrombus formation. They exhibited inhibition of the catalysis of FXa and inhibition of adenosine diphosphate- and U46619- induced platelet aggregation. They also inhibited phosphorylation of myristoylated alanine-rich C kinase substrate induced by ADP- and U46619 and the expression of P-selectin, while activating GP IIb/IIIa 1 in platelets. Improved production of nitric oxide and inhibition of endothelin-1 oversecretion were observed. In vivo studies demonstrated the enhanced antithrombotic effects and elicited anticoagulant effects of the compounds. | ○ | ○ | Article | Structural design of an acoustic planar array transducer by using the equivalent circuit method | Underwater acoustic transducers are used to detect and track targets by using sound waves in underwater environments. To enhance the performance, they can be arranged in various formations to create arrays. However, acoustic interaction between the constituent channels of array transducers is inevitable, and it distorts their characteristics. In this study, the transmitting voltage response (TVR) spectrum of an acoustic planar array transducer is analyzed by considering the crosstalk between channels. The transmitting performance of an array transducer is frequently analyzed using the conventional finite element method (FEM). However, when the size of the array is large, the time and cost of computation using the FEM increase to almost prohibitive levels. To overcome this problem, a new equivalent circuit method (ECM) is developed to analyze the acoustic characteristics of an underwater array transducer. The ECM is validated by comparing its TVR spectrum with that analyzed using the FEM. After validation, the ECM is utilized to design the optimal structure of a planar array transducer having the broadest bandwidth. The sample analysis conducted using the ECM was approximately 1780 times faster than that by the FEM. | ○ | ○ | Article | Apolipoprotein E ε4 genotype and risk of freezing of gait in Parkinson's disease | Objective: To investigate the association between Apolipoprotein E (APOE) genotype and freezing of gait (FOG) in Parkinson's disease (PD). Methods: This cohort study included 339 early PD patients who were divided into APOE 84-positive (n = 88) and 84-negative (n = 251) groups. They were followed-up for up to 6 years to identify the development of FOG. To investigate the influence of CSF beta-amyloid 1-42 (A beta(42)) on the association between APOE 84 and FOG, the patients were additionally dichotomized into "high-level" and "low-level" groups using three different cutoff values for the CSF A beta(42) levels. Results: At baseline, the APOE 84-positive group had lower CSF A beta(42) levels than the APOE 84-negative group. During a median follow-up of 5.0 years, the APOE 84-positive group had a higher incidence of FOG than the APOE 84-negative group. In the multivariable Cox model excluding CSF A beta(42), APOE 84 was a significant predictor of FOG. However, after adding CSF A beta(42) in the model, APOE 84 did not survive, whereas lower CSF A beta(42) levels were associated with FOG. In the subgroup analyses, the effect of the APOE 84 allele was not found in the "lowlevel" group. However, in the "high-level" group, the APOE 84 allele independently increased the risk of FOG, and this association was stronger than the association with CSF A beta(42). Conclusion: The APOE 84 allele may be a novel genetic risk factor for FOG in PD. | ○ | ○ | Article | Objective measurement of limb bradykinesia using a marker-less tracking algorithm with 2D-video in PD patients | Background: Quantitative measurement of parkinsonian motor symptoms is crucial in clinical practice and in research. However, the widely used Unified PD Rating Scale (UPDRS) part III is based on a semi-quantitative evaluation with high interand intra-rater variability. Sensor-based measurements have been widely studied but are limited for their accessibility. Methods: We analyzed 2D-RGB videos recording finger tapping and leg agility tests in 29 PD patients with a marker-less deep-learning based tracking algorithm. The tracking performance was validated with an accelerometer. Four parameters (mean amplitude, mean interpeak interval, amplitude variability and interpeak interval variability) were calculated from the position tracking. Results: The performance of the video-tracking was in good agreement with the accelerometer-based tracking (Intra-class correlation coefficient > 0.9 for the peak amplitude, and >0.6 for the interpeak interval). The video-tracking successfully captured variable aspects of limb bradykinesia that have a distinct correlation with the general parkinsonian motor symptoms and gait. In the finger-tapping task, the mean amplitude (R =-0.6, p = 2.4 x 10(-6)), amplitude variability (R = 0.36, p = 0.0092), mean interpeak interval (R = 0.34, p = 0.014), and interpeak interval variability (R = 0.66, p = 1.4 x 10(-7)) was significantly correlated with the UPDRS scores. In leg agility test, the mean amplitude (R =-0.58, p = 1.7 x 10(-5)), mean interpeak interval (R = 0.37, p = 0.0088) and interpeak interval variability (R = 0.7, p = 6.2 x 10(-8)) were significantly correlated with the UPDRS scores, but not with amplitude variability (R = 0.17, p = 0.26). Limb rigidity was significantly correlated with the interpeak interval (R = 0.40, p = 0.0036) and its variability (R = 0.59, p = 4.2 x 10(-6)) in the leg agility test. | ○ | ○ | Article | Optimal Revascularization Strategy in Non-ST-Segment-Elevation Myocardial Infarction With Multivessel Coronary Artery Disease: Culprit-Only Versus One-Stage Versus Multistage Revascularization | Background Few studies have investigated optimal revascularization strategies in non-ST-segment-elevation myocardial infarction with multivessel disease. We investigated 3-year clinical outcomes according to revascularization strategy in patients with non-ST-segment-elevation myocardial infarction and multivessel disease. Methods and Results This retrospective, observational, multicenter study included patients with non-ST-segment-elevation myocardial infarction and multivessel disease without cardiogenic shock. Data were analyzed at 3 years according to the percutaneous coronary intervention strategy: culprit-only revascularization (COR), 1-stage multivessel revascularization (MVR), and multistage MVR. The primary outcome was major adverse cardiac events (MACE: a composite of all-cause death, nonfatal spontaneous myocardial infarction, or any repeat revascularization). The COR group had a higher risk of MACE than those involving other strategies (COR versus 1-stage MVR; hazard ratio, 0.65; 95% CI, 0.54-0.77; P<0.001; and COR versus multistage MVR; hazard ratio, 0.74; 95% CI, 0.57-0.97; P=0.027). There was no significant difference in the incidence of MACE between 1-stage and multistage MVR (hazard ratio, 1.14; 95% CI, 0.86-1.51; P=0.355). The results were consistent after multivariate regression, propensity score matching, inverse probability weighting, and Bayesian proportional hazards modeling. In subgroup analyses stratified by the Global Registry of Acute Coronary Events score, 1-stage MVR lowered the risk of MACE compared with multistage MVR in low-to-intermediate risk patients but not in patients at high risk. Conclusions MVR reduced 3-year MACE in patients with non-ST-segment-elevation myocardial infarction and multivessel disease compared with COR.
