| ○ | ○ | Article | Antibacterial Effect of Berberrubine on the Salmonella typhimurium Type III Secretion System | The increasing antimicrobial drug resistance of Salmonella typhimurium has been a major public health problem in many countries. The type III secretion system (TTSS) of S typhimurium is an attractive target for the development of new antibiotics against this bacterium. In this study, we determined whether berberrubine derived from Berberis vulgaris could inhibit TTSS expression in S typhimurium. Berberrubine inhibited TTSS expression at a concentration of 60.9 mu M. Additionally, TTSS-related mRNA expression of sipA, sipB, sopA, and sopD was downregulated in the presence of berberrubine. | ○ | ○ | Article | Analysis of the current induced by the motion of a magnet pendulum with a large initial angle | In this study, formulae for the waveform of an induced current, I(t), generated when a magnet traverses along a rectangular coil with a narrow width were theoretically obtained considering the relative motion of the magnet and coil. Consequently, an experiment was performed under experimental conditions that matched the formulas, and the results obtained were compared with the theoretical predictions. The dependence of I(t) on the initial angle empty set (0) of the magnet pendulum and the width w of the rectangular coil was found to beconsistent with the theoretical prediction. | ○ | ○ | Article | Discussion for the enhanced understanding of the photoelectric effect | The photoelectric effect is a crucial phenomenon that contributed to the emergence of modern physics. Even, it is widely used in various fields of physics, such as spectroscopy, image sensing, and particle physics. Albeit its significance in physics, most of the textbooks illuminate a limited perspective focusing on the particle nature of light. This study aims to provide a more productive account of the photoelectric effect from a holistic viewpoint relevant to various fields of physics. This study examines the various fields of physics, from electromagnetics to modern physics, including quantum mechanics such as Fermi-Dirac statistics and light quantum theory. We also suggest extending the quantum mechanics to consider factors influencing the photoelectric effect experiment, such as both anode and cathode materials, geometric considerations of apparatus, and various kinds of interaction between quantum particles. To understand the mechanism of the photoelectric effect deeply, students should be aware of various fields of physics and connect them. | ○ | ○ | Article | Theory and experiment for a solenoid based currents and the magnetic drag | In this study, we derived formulas for the current waveforms induced by a magnet oscillating at arbitrary positions on the axis of a solenoid. We also conducted an experiment for measuring the currents induced in a solenoid by a magnet oscillating in its inner or outer region and compared the results with theoretical predictions. It was confirmed that the magnitude and shape of waveforms of induced currents that were obtained from the derived formulas fit well with experimental results. | ○ | ○ | Article | Convolutional kernel with PrCaMnOx-based resistive random-access memory for neuromorphic image processing | This paper investigated the conductance-state stability of TiN/PrCaMnOx (PCMO)-based resistive random-access memory (RRAM), which serves as a kernel weight element in convolutional neural networks (CNNs), to realize accurate feature extraction from images. On application of the initial forming process that actively drives more oxygen ions to form an interfacial layer between TiN and PCMO to RRAM devices with a high voltage of +/- 4 V, resistive switching behavior with a noticeable memory window was observed. However, the achieved conductance states continued to decrease during repeated cycling. The oxidation at the interface tended to occur thermodynamically, implying an increase in interfacial layer thickness. Considering the hardware implementation of the kernel weight matrix, with specifically assigned conductance values of the RRAM, state instability in the RRAM renders image edge detection difficult, eventually degrading the overall recognition accuracy of the CNN. Thus, we introduced an asymmetric programming voltage method, wherein a higher set voltage of -3 V than a reset voltage of +2.5 V can shift more oxygen ions back into PCMO. Consequently, when the RRAM devices programmed to different states were maintained without degradation in the 1 K cross-point array, eight clearly distinct weighted sum currents were demonstrated in the 3 x 1 subarray. | ○ | ○ | Article | Below-Knee Prosthetic Bypass Is a Viable Option for Limb Salvage in Patients with Extensive Femoropopliteal Occlusive Disease | Purpose: This study aimed to (1) evaluate the outcomes of below-knee prosthetic bypass (BKPB) in the absence of the great saphenous vein, and (2) identify risk factors associated with these outcomes. Materials and Methods: This study included 37 consecutive patients who underwent BKPB with or without distal modification between 2010 and 2022. We further assessed the following treatment outcomes: primary patency (PP), secondary patency (SP), limb salvage (LS), and amputation-free survival (AFS) rates. The risk factors for PP were also examined. Results: Most patients (n=31) were male. In 32 (86.5%) patients, BKPBs were performed for chronic limb-threatening ischemia. At the time of initial admission, two (5.4%) early deaths and three (8.1%) major amputations were noted. At 1 year after BKPB, the overall PP, SP, LS, and AFS rates were 78%, 85%, 85%, and 70%, respectively; at 3 years, they were 58%, 70%, 80%, and 52%, respectively; and at 5 years, they were 35%, 58%, 62%, and 29%, respectively. Notably, PP was significantly lower in limbs with =1 patent tibial arteries than in limbs with >= 2 patent artery ( hazard ratio [HR], 3.80; 95% confidence interval [CI], 1.14-12.69 for overall; and HR, 12.97; 95% CI, 2.15-78.08 for distal anastomosis to below-knee popliteal artery). However, the PP was unaffected by the distal modification. Conclusion: BKPB is a viable option for LS in patients with extensive femoropopliteal disease. | ○ | ○ | Article | Clinical Outcomes of Internal Iliac Artery Interruption during Endovascular Aneurysm Repair | Purpose: This study aimed to investigate the clinical outcomes of internal iliac artery (IIA) interruption during endovascular aneurysm repair (EVAR) and to identify the risk factors for ischemic complications. Materials and Methods: Endovascular treatment was performed in 316 patients with aneurysms or pseudoaneurysms of the abdominal aorta or iliac arteries between March 2006 and January 2022. Medical records and radiological imaging studies were retrospectively reviewed. The incidences of buttock claudication, ischemic colitis, and spinal cord ischemia after IIA interruption were investigated as clinical outcomes. Binary logistic regression analysis were performed to identify the risk factors. Results: IIA embolization was performed in 78 patients. Among the 42 patients who underwent IIA flow preservation procedures, the one-month computed tomography detected early failure in five patients. The origin of the IIA was covered with an endograft in ten patients who did not undergo embolization. Eventually, interruption of the IIA by EVAR was observed in 93 patients. Considering preoperative IIA occlusion, there was a total of six patients who did not have at least one IIA patency. Buttock claudication occurred in 32.6% of the patients, and none of the patients had ischemic colitis or spinal cord ischemia. In multivariable analysis, age <= 80 years and isolated iliac artery aneurysm were associated with the development of postoperative buttock claudication. Conclusion: The most common complication after IIA interruption is buttock claudication; however, critical complications such as ischemic colitis or spinal cord ischemia are rare, even in bilateral IIA occlusion. | ○ | ○ | Article | Effect of Dexmedetomidine Preconditioning on Hepatic Ischemia -Reperfusion Injury in Acute Hyperglycemic Rats | Background. Acute hyperglycemia frequently occurs in stressful situations, including liver transplantation or hepatic surgery, which may affect the protective effects of dexmedetomidine preconditioning and increase postoperative mortality. Therefore, this study aimed to investigate the effects of dexmedetomidine on hepatic ischemia-reperfusion injury in acute hyperglycemia.Methods. Thirty-six Sprague-Dawley rats were randomly assigned to 6 groups, including a combination between 2 glycemic (normo- and hyperglycemia) and 3 ischemia-reperfusion conditions (sham, ischemia-reperfusion only, and dexmedetomidine plus ischemia-reperfusion). Dexmedetomidine 70 mg/kg was preconditioned 30 minutes before ischemic injury. After 6 hours of reperfusion, serum aminotransferase levels were measured to confirm the hepatic tissue injury. Furthermore, inflammatory (nuclear factor-kb, tumor necrosis factor-a, and interleukin-6) and oxidative stress markers (malondialdehyde and superoxide dismutase) were detected.Results. Ischemia-reperfusion injury significantly increased the serum levels of aminotransferase and inflammatory and oxidative stress markers. These ischemia-reperfusion-induced changes were further exacerbated in hyperglycemia and were significantly attenuated by dexmedetomidine preconditioning. However, the effects of dexmedetomidine in hyperglycemia were lesser than those in normoglycemia (P < .05 for aminotransferases, inflammatory markers, malondialdehyde, and superoxide dismutase).Conclusions. These findings suggest that the protective effects of dexmedetomidine preconditioning may be intact against hepatic ischemia-reperfusion injury in acute hyperglycemia. Although its effects appeared to be relatively reduced, this may be because of the increase in oxidative stress and inflammatory response caused by acute hyperglycemia. This is a case of a 36-year-old man who presented with a 1-week history of sudden-onset left leg pain. Diagnostic evaluation, including physical examination, color duplex ultrasonography, computed tomography, and magnetic resonance imaging, revealed a popliteal artery pseudoaneurysm caused by type 5 PAES. The patient underwent surgical exploration to release the entrapment, followed by an interposition graft with the ipsilateral great saphenous vein. Patient postoperative recovery was uneventful, with significant symptomatic improvement. | ○ | ○ | Article | Right Gastroepiploic Artery Transposition for a Common Hepatic Artery and Proper Hepatic Artery Aneurysm Repair | Although hepatic artery aneurysms (HAAs) are uncommon, they are associated with risk of rupture. HAAs >2 cm in diameter require endovascular or open surgical repairs. For HAAs involving the proper hepatic artery or gastroduodenal artery, which is a collateral artery from the superior mesenteric artery, hepatic arterial reconstruction is especially important to avoid ischemic liver injury. In this study, right gastroepiploic artery transposition was performed in a 53-year-old man after a 4 cm common hepatic artery and proper hepatic artery aneurysm was identified. | ○ | ○ | Article | Suspected Malignant Hyperthermia in a Brain-Dead Donor During Anesthesia for Organ Procurement Surgery: A Case Report | We report an unusual case of highly suspected malignant hyperthermia after inducing anesthesia in a brain-dead 18-year-old male patient undergoing organ procurement surgery. The patient was administered desflurane (3 vol%) and rocuronium bromide (50 mg) to induce and maintain general anesthesia. He experienced hypercapnia and tachycardia within 5 minutes of anesthesia induction; however, his body temperature rapidly rose only after 15 minutes. The volatile anesthetic was discontinued, and dantrolene was administered at a low dose (1 mg/kg) to avert possible hepatotoxic effects on the donor liver. Fortunately, the clinical course of the brain-dead donor until the organs were harvested and the liver transplantation outcome of the recipient was favorable. A comprehensive understanding of the pathophysiology of brain death, organ transplantation, and malignant hyperthermia is essential to respond promptly and appropriately. | ○ | ○ | Article | The Association Between Institutional Case Volume of Hematopoietic Stem Cell Transplantation and Mortality | Background. Hematopoietic stem cell transplantation (HSCT) is a complex, high-risk procedure with significant morbidity and mortality. The positive impact of higher institutional case volume on survival has been reported in various high-risk procedures. The association between annual institutional HSCT case volume and mortality was analyzed using the National Health Insurance Service database.Methods. Data on 16,213 HSCTs performed in 46 Korean centers between 2007 and 2018 were extracted. Centers were divided into low-or high-volume centers using an average of 25 annual cases as the cut-off. Adjusted odds ratios (OR) for 1-year mortality after allogeneic and autologous HSCT were estimated using multivariable logistic regression.Results. For allogeneic HSCT, low-volume centers (& LE;25 cases/y) were associated with higher 1-year mortality (adjusted OR 1.17, 95% CI 1.04-1.31, P = .008). However, low-volume centers did not show higher 1-year mortality (adjusted OR 1.03, 95% CI 0.89-1.19, P = .709) for autologous HSCT. Long-term mortality after HSCT was significantly worse in low-volume centers (adjusted hazard ratio [HR] 1.17, 95% CI, 1.09-1.25, P < .001 and adjusted HR 1.09, 95% CI, 1.01-1.17, P = .024, allogeneic and autologous HSCT, respectively) compared with high-volume centers.Conclusion. | ○ | ○ | Article | Type Ic Endoleak after LifeStream Balloon-Expandable Stent Graft and Zenith Iliac Branch Device Placement | Considering the recent advancements in endovascular management of aortoiliac aneurysms, the use of an iliac branch device (IBD) to preserve pelvic blood flow and reduce complications caused by embolization of the internal iliac artery (IIA) is recommended by various guidelines. Although the outcomes reported following IBD placement are mainly positive and durable, IBD-specific complications such as a type Ic endoleak and associated reintervention may occur. Moreover, only one IBD device and one type of balloon-expandable bridging stent graft for IIA are currently available on the domestic market. Here we present two cases of type Ic endoleak following IBD placement. In both cases, IIA diameter was slightly larger than the basic instructions for use. Notably, the initial procedures were considered successful; however, type Ic endoleaks were detected on 1-month follow-up imaging. | ○ | ○ | Article | Effect of gradation on shear characteristics of granular materials under one-way repeated direct shear and simple shear loadings | The shear strength and stiffness of granular materials are crucial parameters for evaluating both stability and sustainability in geoenvironmental engineering. Two types of shear loading, static and cyclic, may directly influence the mechanical properties of granular materials; however, the investigation of stiffness improvement of granular materials has not yet been sufficiently investigated using a loading method. The one-way repeated loading method described in this study may overcome the drawbacks of classical testing approaches. One-way repeated direct shear (DS) and direct simple shear (DSS) tests were conducted on a series of poorly graded bead gradations to investigate the shear strength and stiffness of granular materials under a drained and strain-controlled condition with a shear velocity of 0.1 mm/min. All samples in the two tests were prepared using the same method and initial geometry. Three types of samples referring to different curvature coefficient (C-u) values of 1.8, 3.3, and 5.8 and normal stresses (& sigma;(n)) of 50, 100, and 150 kPa were applied. The test results show that an increase in C-u results in an increase in the shear stress of the granular materials at the peak and ultimate states for both the DS and DSS tests. The peak shear stress ratios between the two tests (& tau;(DSS)/& tau;(DS)) ranged from 0.75 to 0.95 when C-u and & sigma;(n) were increased. The angles of shearing resistance from the DS test were in a wide range of 25.7 & DEG;-33.7 & DEG; at the peak state and 24.9 & DEG;-31.3 & DEG; at the ultimate state, whereas those found in the DSS test were in narrow ranges of 26.0 & DEG;-28.4 & DEG; and 23.6 & DEG;-27.9 & DEG; at the peak and ultimate states, respectively. It should be noted that an increase in C-u led to the increase in the stiffness ratio in both test methods. The difference in stiffness ratios determined from the DS and DSS tests was approximately 14%.
