| ○ | ○ | Article | NEWLY DISCOVERED z ∼ 5 QUASARS BASED ON DEEP LEARNING AND BAYESIAN INFORMATION CRITERION | We report the discovery of four quasars with M-1450 greater than or similar to -25.0 mag at z similar to 5 and supermas-sive black hole mass measurement for one of the quasars. They were selected as promising high-redshift quasar candidates via deep learning and Bayesian information criterion, which are expected to be effective in discriminating quasars from the late-type stars and high-redshift galaxies. The candidates were observed by the Double Spectrograph on the Palomar 200-inch Hale Telescope. They show clear Ly alpha breaks at about 7000-8000 A, indicating they are quasars at 4.7 < z < 5.6. For HSC J233107-001014, we measure the mass of its supermassive black hole (SMBH) using its C Iv lambda 1549 emission line. The SMBH mass and Eddington ratio of the quasar are found to be similar to 10(8) M-circle dot and similar to 0.6, respectively. This suggests that this quasar possibly harbors a fast growing SMBH near the Eddington limit despite its faintness (L-Bol < 10(46) erg s(-1)). | ○ | ○ | Article | Ras-related proteins (Rab) play significant roles in sperm motility and capacitation status | Rab proteins are widely known for their involvement in establishing Golgi apparatus and controlling Golgi trafficking in eukaryotic cells. Specifically, Rab proteins play significant roles in acrosome formation and exocytosis. Furthermore, mechanisms involved in the regulation of Rab proteins during capacitation have been identified. However, there has been no direct evaluation to assess the correlation between Rab proteins and sperm function. Consequently, this study was designed to analyze the correlation between Rab proteins and sperm functions. Individually, we analyzed the sperm motility patterns, motion kinematics, capacitation status, and Rab protein expression levels of sperm samples from 31 boars before and after capacitation. As a result, we discovered that Rab3A, Rab5, Rab11, Rab14, and Rab27A correlated with various sperm motility patterns, motion kinematics before capacitation. Rab3A, Rab5, Rab11, Rab14, and Rab34 correlated with various sperm motility patterns, motion kinematics after capacitation. Moreover, Rab4 and Rab34 were associated with capacitation status before capacitation, and Rab3A, 25, and 27A correlated with capacitation status after capacitation. This is the first study to analyze the correlation between Rab proteins and sperm functions. Collectively, our results indicate that specific sperm motility and kinematics, as well as the structural condition of the sperm head and capacitation status, regulate individual Rab protein. | ○ | ○ | Article | RELATION BETWEEN BLACK HOLE MASS AND BULGE LUMINOSITY IN HARD X-RAY SELECTED TYPE 1 AGNs | Using I-band images of 35 nearby (z < 0.1) type 1 active galactic nuclei (AGNs) obtained with Hubble Space Telescope, selected from the 70-month Swift-BAT X-ray source catalog, we investigate the photometric properties of the host galaxies. With a careful treatment of the point-spread function (PSF) model and imaging decomposition, we robustly measure the I-band brightness and the effective radius of bulges in our sample. Along with black hole (BH) mass estimates from single-epoch spectroscopic data, we present the relation between BH mass and I-band bulge luminosity (M-BH-M-I,M- bul relation) of our sample AGNs. We find that our sample lies offset from the M-BH-M-I,M- bul relation of inactive galaxies by 0.4 dex, i.e., at a given bulge luminosity, the BH mass of our sample is systematically smaller than that of inactive galaxies. We also demonstrate that the zero point offset in the M-BH-M-I,M- bul relation with respect to inactive galaxies is correlated with the Eddington ratio. Based on the Kormendy relation, we find that the mean surface brightness of ellipticals and classical bulges in our sample is comparable to that of normal galaxies, revealing that bulge brightness is not enhanced in our sample. | ○ | ○ | Article | SOLAR ACTIVITY AND LATITUDINAL DISTRIBUTION OF SUNSPOTS | We explore the latitudinal distribution of sunspots and pursue to establish a correlation between the statistical parameters of the latitudinal distribution of sunspots and characteristics of solar activity. For this purpose, we have statistically analyzed the daily sunspot areas and latitudes observed from May in 1874 to September in 2016. As results, we confirm that the maximum of the monthly averaged International Sunspot Number (ISN) strongly correlates with the mean number of sunspots per day, while the maximum ISN strongly anti-correlates with the number of spotless days. We find that both the maximum ISN and the mean number of sunspots per day strongly correlate with the the average latitude, the standard deviation, the skewness of the the latitudinal distribution of sunspots, while they appears to marginally correlate with the kurtosis. | ○ | ○ | Article | Types of social activities associated with fewer insomnia symptoms and the mediating role of loneliness in older adults | Background A positive association has been known to exist between social activity engagements and fewer insomnia symptoms in later life. However, little is known about which social activities are associated with insomnia symptoms. Investigating mediating factors in this relationship may contribute to developing effective strategies for the reduction of sleep complaints in older adults. This study aimed to examine the mediating role of loneliness on the relationship between engagement in different social activities and insomnia symptoms. Methods We used secondary data from the 2018 Health and Retirement Study. The study sample included 3236 older adults who responded to a survey on social activity engagement, insomnia symptoms, and loneliness. After adjusting for covariates, simple mediation analyses were performed with bootstrapping to identify the mediating role. Results Among the several types of social activities, higher levels of engagement in educational courses and community arts group had a significant and direct relationship with fewer insomnia symptoms in older adults. Mediation analyses confirmed the mediating role of loneliness in the relationship between these specific social activity engagements and insomnia symptoms. | ○ | ○ | Article | Human-robot-robot cooperative control using positioning robot and 1-DOF traction device for robot-assisted fracture reduction system | While performing musculoskeletal long bone fracture reduction surgery, assistant surgeons can often suffer from physical fatigue as they provide resistance against the tension from surrounding muscles pulling on the patient's broken bones. These days, robotic systems are being actively developed to mitigate this physical workload by realigning and holding these fractured bones for surgeons. This has led to one consortium proposing the development of a robot-assisted fracture reduction system consisting of a 6-DOF positioning robot along with a 1-DOF traction device. With the introduction of the 1-DOF traction device, the positioning robot does not have to fight these contraction forces so can be compact improving its maneuverability and overall convenience; however, considering surgeon-robot interactions, this approach adds the requirement of controlling two different types of robots simultaneously. As such, an advanced cooperative control methodology is required to control the proposed bone fracture reduction robot system. In this paper, a human-robot-robot cooperative control (HRRCC) scheme is proposed for collaboration between the surgeon, the positioning robot, and the traction device. First, the mathematical background of this HRRCC scheme is provided. | ○ | ○ | Article | Implementation and Performance Analysis of a Non-codebook Based MU-MIMO System with Dynamic Precoding for TDD LTE-Advanced | This paper presents a non-codebook based Multiple User Multiple Input Multiple Output (MU-MIMO) test-bed system for Time Division Duplex (TDD) Long Term Evolution-Advanced (LTE-A). The system was implemented for verifying the performance of a dynamic precoding procedure that selects the most appropriate precoder among a given set of precoding techniques. Using two parameters, the propagation path gain and the condition number of the channel matrix, the proposed MU-MIMO system can adaptively switch the precoding software to guarantee that the upper bound Bit Error Rate (BER) is maintained with a minimum computational burden. This paper also presents a novel procedure for accurately and simply calibrating the multiple Radio Frequency (RF) paths of the multiple antennas and RF transceivers of the MU-MIMO system. From various experimental measurements obtained from the implemented test-bed system, the upper bound BER, which was arbitrarily set to 10(-3) in this paper, can be maintained with the simplest precoder, Zero Forcing (ZF), unless the propagation path gain becomes less than 0.25. | ○ | ○ | Article | Efficacy of sigmoidoscopy for evaluating disease activity in patients with ulcerative colitis | Background Endoscopic assessment of disease activity is a key parameter in the management of ulcerative colitis. Whether sigmoidoscopy alone is sufficient to evaluate the disease activity in ulcerative colitis lacks studies. Methods We retrospectively analyzed the medical records and endoscopic results of patients with ulcerative colitis followed by colonoscopy in seven tertiary hospitals between January 2012 and December 2018. Endoscopic disease activity was scored using the Mayo endoscopic subscore (MES) and Ulcerative Colitis Endoscopic Index of Severity (UCEIS) for each segment from the colonoscopy images. Concordance was evaluated by comparing the highest MES and UCEIS in the rectosigmoid and proximal regions to confirm the usefulness of sigmoidoscopy. Results A total of 500 colonoscopic examinations from 333 patients were enrolled. Only in 7.6% [k(kappa): 0.893, r(Spearman): 0.906, p = 2) in the rectosigmoid area and the entire colon showed a high concordance rate [k(kappa): 0.899, r(Spearman): 0.904, p < 0.001]. Endoscopic healing (MES = 0) also showed a high concordance rate [k(kappa): 0.882, r(Spearman): 0.887, p < 0.001]. In 38 cases (7.6%) of patients with a higher MES in the proximal area, it was significantly higher in patients with previous extensive colitis. Conclusions Sigmoidoscopy and colonoscopy showed a high concordance rate. Therefore, sigmoidoscopy is considered a sufficient substitute for colonoscopy. | ○ | ○ | Article | Accidental esophageal intubation via a large type C congenital tracheoesophageal fistula: A case report | BACKGROUND Tracheoesophageal fistula (TEF) is a congenital anomaly characterized by interruptions in esophageal continuity with or without fistulous communication to the trachea. Anesthetic management during TEF repair is challenging because of the difficulty of perioperative airway management. It is important to determine the appropriate position of the endotracheal tube (ETT) for proper ventilation and to prevent excessive gastric dilatation. Therefore, the tip of the ETT should be placed immediately below the fistula and above the carina. CASE SUMMARY A full-term, one-day-old, 2.4 kg, 50 cm male neonate was diagnosed with TEF type C. During induction, an ETT was inserted using video laryngoscope and advanced deeply to ensure that the tip passed over the fistula, according to known strategies. The passage of the ETT through the vocal cords was confirmed via video laryngoscope. However, after inflating the ETT cuff, breath sounds were not heard on bilateral lung auscultation. Instead, gastric sounds were heard. Considering that a large fistula (approximately 6.60 mm x 4.54 mm) located 10.2 mm above the carina was confirmed on preoperative tracheal computed tomography, the possibility of unintentional esophageal intubation was highly suspected. Therefore, we decided to uncuff and withdraw the ETT carefully for repositioning, while monitoring auscultation and end-tidal CO2 simultaneously. At a certain point (9.5 cm from the lip), clear breath sounds and proper end-tidal CO2 readings were suddenly achieved, and adequate ventilation was possible. | ○ | ○ | Article | Extracorporeal membrane oxygenation for lung cancer-related life-threatening hypoxia: A case report | BACKGROUNDLife-threatening hypoxia can occur in patients with lung cancer due to bronchial obstruction. Extracorporeal membrane oxygenation (ECMO) can be used as a bridge therapy for patients with severe hypoxia not relieved by conventional mechanical treatment. However, the usefulness of chemotherapy in patients with lung cancer receiving ECMO therapy is not well known. CASE SUMMARYA 53-year-old man visited the emergency room with worsening dyspnea for 1 mo. A series of imaging and diagnostic tests were performed, and stage IIIB (cT4N2M0) lung cancer was eventually diagnosed. On hospital day 3, he experienced dyspnea and hypoxia that was not relieved with oxygen support via a high-flow nasal cannula. ECMO was initiated because his respiratory condition did not improve even with mechanical ventilation. The patient then underwent gemcitabine/cisplatin chemotherapy without dose reduction while on ECMO. After two cycles of chemotherapy, there was a decrease in the size of the primary tumor in the right main bronchus. After the completion of concurrent chemoradiotherapy, a computed tomography scan revealed further improvement in the right main bronchus narrowing. Eight months after a lung cancer diagnosis, the patient did well without any dyspnea. | ○ | ○ | Article | Large retroperitoneal atypical spindle cell lipomatous tumor, an extremely rare neoplasm: A case report | BACKGROUND Atypical spindle cell lipomatous tumor (ASLT) is a rare soft tissue neoplasm with a low potential for malignancy. ASLT frequently occurs in the limb and limb girdles. However, large retroperitoneal ASLTs are extremely rare. There was no concrete case report of retroperitoneal ASLTs. CASE SUMMARY An 18-year-old woman presented with abdominal pain and a palpable mass. Abdominal computed tomography revealed a large fatty mass that was approximately 30 cm in size and filled the entire abdominal cavity. Surgical excision was indicated. The tumor did not invade the adjacent organs. The pelvic cavity was then too narrow to dissect smoothly. The mass was successfully excised without tumor rupture or adjacent organ injury. Microscopically, the neoplasm was a well-differentiated adipocy tic neoplasm. Immunohistochemical staining showed that the spindle cells were positive for CD34 and desmin, in addition to multifocal positivity for S100 protein. These histological features were consistent with an ASLT. The patient's postoperative course was uneventful. At the 12-mo follow-up, no evidence of recurrence or metastasis was observed. CONCLUSION To the best of our knowledge, our study is the first concrete report of a large retroperitoneal ASLT in the English literature. In the large retroperitoneal ASLT located in the pelvic cavity, which made it too narrow and tight to dissect, complete excision is difficult but very important because of recurrence risk. Al though large retroperitoneal ASLTs are considered extremely rare., their detection is important for accurate evaluation and management. | ○ | ○ | Article | Management of symptomatic cervical facet cyst with cervical interlaminar epidural block: A case report | BACKGROUNDSymptomatic cervical facet cysts are relatively rare compared to those in the lumbar region. These cysts are usually located in the 7(th) cervical and 1(st) thoracic vertebral (C7/T1) area, and surgical excision is performed in most cases. However, facet cysts are associated with degenerative conditions, and elderly patients are often ineligible for surgical procedures. Cervical interlaminar epidural block has been used in patients with cervical radiating symptoms and achieved good results. Therefore, cervical interlaminar epidural block may be the first-choice treatment for symptomatic cervical facet cysts.CASE SUMMARYA 70-year-old man complained of a tingling sensation in the left hand, focused on the 4(th) and 5(th) fingers, for 1 year, and posterior neck pain for over 5 mo. The patient's numeric rating scale (NRS) score was 5/10. The patient was diagnosed with symptomatic cervical facet cyst at the left C7/T1 facet joint. Fluoroscopy-guided cervical interlaminar epidural block at the C7/T1 level with 20 mg triamcinolone and 5 mL of 0.5% lidocaine was administered. The patient's symptoms improved immediately after the block, with an NRS score of 3 points. | ○ | ○ | Article | Monitored anesthesia care for craniotomy in a patient with Eisenmenger syndrome: A case report | BACKGROUND Eisenmenger syndrome ( ES) is an uncorrected congenital cardiac defect with a left-to-right shunt, leading to pulmonary arterial hypertension. Patients with ES are susceptible to hemodynamic alterations during noncardiac surgery with general anesthesia, which increases perioperative morbidity and mortality. Monitored anesthesia care (MAC) is often used during minor procedures in patients with cardiac disease. However, few reports on MAC in patients with ES exist. CASE SUMMARY A 49-year-old man was admitted for a severe headache lasting 30 d. He had been diagnosed with a large perimembranous ventricular septal defect ( VSD) with bidirectional shunt flow and pulmonary arterial hypertension 10 years ago. A round mass in the right frontal lobe was revealed by Magnetic resonance imaging. Stereotactic aspiration using a neuronavigation system was performed under MAC. The patient was stayed in the hospital for 5 d, and discharged without complications. | ○ | ○ | Article | Pressure changes in tapered and cylindrical shaped cuff after extension of head and neck: A randomized controlled trial | BACKGROUND The proper cuff pressure of endotracheal tube (ET) plays an important role in sealing the airway and preventing airway complications during mechanical ventilation. The ET cuff shape affects the cuff pressure after positional change. AIM To investigate cuff pressure between tapered and cylindrical cuff after extension of head and neck during nasal endotracheal intubation. METHODS In a randomized clinical trial, 52 patients were randomized to one of two groups: cylindrical cuff or Tapered cuff. Cuff pressure with 22 cmH(2)O was applied to patients in the neutral position. After extension of head and neck, the cuff pressure was evaluated again and readjusted to 22 cmH(2)O. In addition, the extent of cephalad migration of ET tip was assessed and postoperative airway complications such as sore throat, and hoarseness were measured. RESULTS The cuff pressure was higher in the tapered cuff (28.7 +/- 1.0 cmH(2)O) than in the cylindrical cuff (25.5 +/- 0.8 cmH(2)O) after head and neck extension (P < 0.001). The extent of cephalad migration of tube tip was greater in TaperGuard ET (18.4 +/- 2.2 mm) than in conventional ET (15.1 +/- 1.2 mm) (P < 0.001). The incidence of postoperative airway complications was comparable between two groups. | ○ | ○ | Article | Changes in the flexor pollicis longus tendon during spontaneous resolution of pediatric trigger thumb | The present study aimed to evaluate conformational changes in the flexor pollicis longus (FPL) tendon and inner space of the A1 pulley during spontaneous resolution of pediatric trigger thumb (PTT) and to compare them versus the normal contralateral side. We enrolled 36 patients with unilateral PTT who underwent ultrasonography twice between January 2016 and July 2020 and showed > 108 improvement in thumb interphalangeal (IP) joint extension lag. Ultrasonography was used to measure the anteroposterior (AP) and radioulnar (RU) diameters of the FPL tendon proximal to the A1 pulley and of the inner space of the A1 pulley. On the side of the PTT, the average extension lag in the thumb IP joint significantly improved, from 35.68 to 14.28. The AP and RU diameters of the FPL tendon increased by 0.3% and 1.9%, respectively, and those of the inner space of the A1 pulley increased by 15.3% and 5.0%, respectively. In the contralateral normal thumb, the AP and RU diameters of the FPL tendon increased by 12% and 9.3%, respectively, and those in the inner space of the A1 pulley increased by 9.9% and 4.6%. During improvement in IP joint extension lag, the mismatch between the enlarged FPL tendon and the inner space of the A1 pulley was reduced by their asymmetric growth.
