Article Bi-directional evolutionary 3D topology optimization with a deep neural network The FEM-based topology optimization repeats usually finite element analyses many times to converge to the stopping criteria. If the near-optimal topology data are available in advance at the beginning of an optimization process, the iterative computation could be greatly reduced. In an effort to obtain swiftly optimum topology solutions, the deep learning and neural networks with a special segmentation scheme of digital images are combined with the BESO (bi-directional evolutionary structural optimization) topology method in this study. The pre-trained digital images of 3200 optimum topologies construct the design domain for the main topology optimization. Additionally, a new post-processor is developed in order to reconstruct the relative locations among finite elements in the raw outputs generated by the neural network. The proposed method has been demonstrated to be efficient in lowering the iterations with several 2D and 3D optimization examples. The iteration counts can be reduced 63% for a 2D example and by 72.5% for a 3D one, compared to BESO results alone. Article Clinical Utility of Next-generation Sequencing in Real-world Cases: A Single-institution Study of Nine Cases Background/Aim: Targeted next-generation sequencing (NGS) is a well-established technique to detect pathogenic alterations in tumors. Indeed, it is the cornerstone of targeted therapy in precision medicine. We investigated the clinical utility of next-generation sequencing in real-world cases. Patients and Methods: We retrospectively selected six representative cancer cases, wherein targeted NGS played a pivotal role in the diagnosis and treatment of patients. Additionally, we analyzed three cases with rare, unusual pathogenic alterations. Results: Our NGS analysis revealed that four patients had TPR-ROS1, EGFR-RAD51, and NCOA4-RET fusions and MET exon 14 skipping mutation, respectively, which can be treated with targeted therapy. Furthermore, we used NGS as a diagnostic tool to confirm the origin of unknown primary malignant tumors in two cases. Interestingly, NGS also helped us identify the following cases: patients exhibiting BRCA1 and TP53 mutations that exhibited histological and immunohistochemical characteristics consistent with endometrioid carcinoma, patients with high-grade serous carcinoma not possessing a TP53 mutation, and patients with small cell lung cancer with a ERBB2 mutation and displaying no loss of RB1. Conclusion: We recommend targeted NGS for the diagnoses and targeted therapy of cancer patients. Article Comparison of Survival Outcomes According to BRCA1/2 Variant Type in High-grade Serous Ovarian Cancer Background/Aim: Mutations of BRCA1/2 improve cancer prognosis due to their better response to platinum based chemotherapy. This study evaluated overall survival (OS) and progression-free survival (PFS) under similar conditions of first-line adjuvant chemotherapy within seven years in high-grade serous ovarian cancer (HGSOC). Patients and Methods: A total of 160 patients were enrolled. The pathogenic BRCA1/2 variant group included pathogenic variant and likely-pathogenic variant, while the nonpathogenic group included wild-type and variant of uncertain significance. For first-line chemotherapy, delivered dose intensity, relative dose intensity, and delay of duration were calculated in all patients. Results: Of the tested variants, 108 (67.5%) were non-pathogenic and 52 (32.5%) were pathogenic. No significant difference was found in various clinical factors of cancer stage, surgery, or chemotherapy. There was no significance for OS or PFS within five or seven years. Article Drag reduction for flow past a square cylinder through corner chamfering This study investigates the unsteady incompressible flow around a square cylinder with different chamfer ratios (CRs) using a commercial finite volume code, ANSYS Fluent. CR ranges from 0.0 (sharp square cylinder) to 0.5 (diamond cylinder) with variable increments. Detailed analysis of flow characteristics is conducted at Reynolds number (Re) = 2100. Additionally, simulation is extended to cover Re, i.e., Re = 100, 500, and 10000. The simulation results show that cylinder with CR = 0.1 outperforms all other cases by enabling a drag reduction of about 60 % at Re = 10(4). Drag has an inverse relationship with the wake closure length. Time-averaged coefficient of pressure, streamlines, and vorticity contours are also discussed to better understand near-wake features and the physics of drag reduction. Article Effect of a large pulsed electron beam (LPEB) irradiation on mechanical properties and fatigue behavior of Ti-6Al-4V The large pulsed electron beam (LPEB) is recently highlighted as a surface modification method of metallic alloys owing to various advantages such as high thermal gradient and fast processing speed compared to conventional surface treatments. Besides its effect on surface properties, the effect of LPEB irradiations of fatigue behavior of metallic alloys is yet to be explored. In this study, effects of the LPEB irradiation on fatigue behavior of Ti-6Al-4V titanium alloy were investigated. Article Effect of Breast Silicone Implant Topography on Bacterial Attachment and Growth: An In Vitro Study Background/Aim: The mechanisms underlying capsular contracture remain unclear. Emerging evidence supports the inflammation hypothesis, according to which bacteria from an adherent biofilm cause chronic inflammation and collagen deposition on the implant and trigger capsular contracture. Our goal was to evaluate the effect of different types of breast implants on the growth of Staphylococcus aureus, S. epidermidis, and Pseudomonas aeruginosa, which are commonly found in biofilms in infection. Materials and Methods: Bacteria were grown in tryptic soy broth at 37??C for 2, 6, and 24 h and subsequently incubated for 24 h on 12 shell sections of smooth, nano-, and macrotextured breast implants. After incubation, the solutions were ultrasonicated and bacterial numbers were determined by serial dilution. S. aureus were fixed, washed with phosphate-buffered saline, dehydrated in ethanol, and coated with a platinum film to visualize the presence of biofilms by scanning electron microscopy. Article Effects of Methylsulfonylmethane on UVB-induced Skin Damage: An Experimental Study in a Mouse Model Background/Aim: The skin protects the body from ultraviolet rays and other external factors. Various studies have been conducted to identify methods to prevent skin aging and damage. To investigate the protective effects of methylsulfonylmethane (MSM), in this study, a hairless mouse model was used. Patients and Methods: Mice divided into Groups B, C, and D were subjected to UVB irradiation for six weeks, and Group A was considered the control. Retinoic acid is a substance that has been proven to have anti-aging properties. Group C was injected with MSM, group D was injected with retinoic acid, and groups A and B were injected with saline. At the end of the experiment, the degree of senescence was confirmed through visual evaluation, histopathological analysis, immunohistochemistry, and elasticity measurement using SEM. Results: After the end of the experiment, the wrinkle score was 0.4, 2.5, 1.8, 1.5 for Groups A, B, C, and D, respectively. Epidermal thickness was 40 mu m, 70 mu m, 60 mu m, 55 mu m in Groups A, B, C, and D, respectively. Group C showed less collagen confirmation loss and more angiogenesis and elastin precursor production. Elastic fiber linearity was 0.901 +/- 0.02, 0.551 +/- 0.04, 0.751 +/- 0.04, 0.822 +/- 0.03 for Groups A, B, C, and D, respectively. Conclusion: Injection of MSM in mice subjected to UVB-induced skin damage reduces the wrinkle score and protects against photoaging. Article Efficient dynamic behavior analysis of an adhesive-jointed structure made of composites An inefficiency issue in substructure analysis is caused by a finite element model of an adhesive interface meshed into thousands of degrees of freedom. This paper presents a wave-approach based adhesive interface for efficient dynamic behavior analysis of a large adhesive jointed composite structure. The large adhesive interface is converted into a basis function known as waves. Orthonormalization selects the most important waves and reduces the interface size. Two engineering cases were presented. The predicted results obtained from the approach were compared with the full-size finite element and experimental counterparts to assess their accuracy level. It was discovered that the approach could significantly reduce the need for repeated long-term composite structure optimization analysis. Article Energy Efficiency Prediction Model of Heavy-duty Electric Vehicles Using Numerical Simulation This paper is a study on energy efficiency prediction model of heavy-duty electric vehicles using numerical simulation. Because the current energy consumption efficiency evaluation method for electric vehicles has limitations in terms of manpower, resources, and time required, therefore a simulation model development to predict electric vehicle energy consumption efficiency is needed. Furthermore, research on heavy-duty electric vehicles is relatively insufficient compared to light-duty electric vehicles, and the need of research on heavy-duty electric vehicles is increasing. Therefore, this study develops an energy efficiency prediction model for heavy-duty electric vehicles and verifies it with the experimental results. Based on this, the applicability of the simulation method using the developed prediction model to the current energy efficiency evaluation management system for heavy-duty electric vehicles was reviewed. To verify the accuracy of the simulation model, the dynamometer test results were compared to simulation results, and the UDS protocol was used to acquire internal data for heavy-duty electric vehicles. Reliability of simulation model was secured by comparing data such as motor speed, battery voltage/current, state of charge and total driving distance of the test vehicles with data from the dynamometer experiment and the simulation model. Article Genomic Profiling of Aggressive Thyroid Cancer in Association With its Clinicopathological Characteristics Background/Aim: Poorly differentiated thyroid carcinoma (PDTC), anaplastic thyroid carcinoma (ATC), and advanced DTC have poor outcomes. Materials and Methods: We performed next-generation sequencing in nine selected aggressive thyroid cancers. Results: Among the nine patients, the driver gene mutations BRAF V600E (3/9) and NRAS Q61K (1/9) were detected. Other oncogenic mutations included ERBB2 (1/9) and CDK4 (1/9). Telomerase reverse transcriptase (TERT) promoter mutation was found in five cases. Among tumor suppressor genes, mutations in TP53 (3/9), ARID1A (1/9), APC (1/9), MEN1 (1/9), DICER1 (1/9), and MED12 (1/9) were identified. RET fusions were found in two cases, one with PTDC and the other with ATC. The ATC with RET fusion also harbored TP53 and TERT promoter mutations. None of the PDTC cases had BRAF or RAS gene alterations. Conclusion: Since genetic alterations with therapeutic and prognostic implications were detected using next-generation sequencing, this technique is recommended to be performed for patients with aggressive thyroid cancer. Article Investigation of constantly and transiently propagating cracks in functionally graded materials by dynamic photoelasticity In this study, the stress intensity factors (SIFs) for steady and transient propagation of cracks in transparent homogeneous functionally graded materials were analyzed by using the photoelasticity technique. The fracture analysis was carried out for the cracks propagating from a region with high elasticity towards low elasticity, as well as the cracks propagating from a region with low elasticity towards high elasticity. The analysis includes cracks propagating (1) at an almost steady speed, and (2) with the rapid increase, followed by a decrease in speed. For cracks with almost constant velocity, the SIFs were greater when a crack started from a high elasticity region, as compared to the cracks which initiated from a low elasticity region. For cracks propagating with rapid acceleration and deceleration, when the strain energy accumulated in the material due to an increase in load or stress was released at the moment of crack propagation, the SIF was momentarily lowered by approximately 45 %-50 % of the static SIF(before crack initiation), which subsequently increases by approximately 30 % eventually, the crack acceleration approaches nearly zero; the SIF decreases and increases respectively as the crack propagates in a material with decreasing and increasing modulus of elasticity. Article Long-term Experience of Single-port and Multi-port Total Laparoscopic Hysterectomy in a Single Center Background/Aim: Short-term feasibility and safety of single-port total laparoscopic hysterectomy (TLH) compared with conventional multi-port TLH have been previously demonstrated. However, recent studies are insufficient. This study aimed to analyze long-term data from our center to examine the current status of single-port and multi-port TLH.Patients and Methods: In 766 patients who received TLH from 2005 to 2019, 325 were single-port and 441 were multi-port. Inclusion criteria were benign and premalignant uterine diseases. To reduce the impact of treatment selection bias and potential confounding factors, inverse probability of treatment weighting was applied. Results: Single-port TLH showed significantly better clinical outcomes for hospital stay, operative time, hemoglobin decrease, and complication rate than the multi-port TLH after correction of biases. C-reactive protein increased after the single-port TLH. Conclusion: As laparoscopic surgical instruments and surgeon's skill have been improved, some surgical outcomes have become significantly better in single -port TLH after long-term observation. Article Patterns-Based Coating of Piston Skirt for Added Benefits during Hydrodynamic Lubrication in Internal Combustion Engines Friction has been a significant research concern for decades. Most of the energy input into an internal combustion (IC) engine is wasted because it is used to overcome friction. Scientific developments have been made to reduce friction, including surface coatings and surface textures, which have emerged as potential solutions to reducing friction throughout the lubricating regimes. This study combines surface texturing with the coating approach to investigate the frictional response. Realistic tribo-dynamics (secondary motion of the piston skirt) are considered during actual engine-like conditions to explore the effects of pattern-based coating on friction in hydrodynamic lubrication. Results show a remarkable reduction in friction and encourage the pattern-based development of surface coatings to extend the benefit across lubricating regimes. Consequently, the surface coated with optimized patterns had 7.98 +/- 0.34 % reduced energy loss per engine cycle as compared with the surface coated without a pattern. Article Process design of multi-stage cold forging with small size for ESC solenoid valve parts ESC's solenoid valves independently brake each wheel, allowing the vehicle to maintain braking and directional stability. The most important component of the solenoid valve is the armature manufactured by the turning process by a CNC lathe so far. In this study, we propose to use the multi-stage cold forging process as an alternative to reduce manufacturing cost while satisfying dimensional accuracy and performance. To apply the multi-stage cold forging process, the whole forming process is divided into six stages to improve the dimensional accuracy of the outer diameter, full length and slot part of the armature. It is divided into 6 stages, but 6 stages are operated simultaneously in one stroke of forging. Inducing uniform plastic deformation by designing the preform for each stage of the armature slot. Through the proposed process design for the multi-stage cold forging process, the dimensional accuracy of the test product can be improved. The weight of the input material in the turning process can be reduced by 46 % from 3.5 g to 1.9 g by applying the multi-stage cold forging process. In addition, the manufacturing process can be reduced from 2 processes to 1 process, and the production quantity can be increased by 2500 % from 180 ea to 4800 ea per hour. Article Survival Outcomes With Reduced Doses of Adjuvant Chemotherapy in Advanced Epithelial Ovarian Cancer Background/Aim: There have not been enough recent studies investigating the incidence or efficacy of dose reduction in adjuvant chemotherapy for epithelial ovarian cancer. This study examined whether patients who needed dose reduction showed poorer survival outcomes. Patients and Methods: From 2011 to 2021, 102 patients were included in the study. Patients who underwent neoadjuvant chemotherapy and those with early-stage disease were excluded. Patients were divided into two groups: those who had a 60%. Of the 102 patients, 38 (37.3%) underwent dose reduction 60%. Results: PFS was significantly longer in the group whose dose reductions were >60%, whereas OS was not significant. Conclusion: A dose reduction of >60%, determined by patients??? medical conditions, during first-line of adjuvant chemotherapy does not negatively influence survival outcomes, such as OS and PFS, in advanced epithelial ovarian cancer.
