Article The Association of Childhood Maltreatment with Adulthood Mental Disorders and Suicidality in Korea: a Nationwide Community Study Background: Although childhood maltreatment is a known risk factor for adulthood mental health, the impact of different types of childhood maltreatment on mental disorders is not yet clear. This study explored the association of each type of childhood maltreatment with adulthood mental disorders and suicidality in South Korea. Methods: A total of 5,102 individuals from the general populations over the age of 18 responded to the Korean version of the Composite International Diagnostic Interview and questions about childhood maltreatment (emotional neglect, psychological abuse, physical abuse, and sexual abuse). To evaluate the odds ratio for mental disorders and suicidality associated with each type of childhood maltreatment, we used logistic regression analysis. Results: About 17.0% of the respondents reported having experienced a type of maltreatment in childhood. According to the type, 9.4% reported physical abuse, 9.3% reported emotional neglect, 7.9% reported psychological abuse, and 3.8% reported sexual abuse. Exposure to each type of childhood maltreatment was associated with most types of mental disorders after adjusting for sociodemographic factors. Each type of childhood maltreatment victim was associated with suicidality (suicidal ideations, suicide plans, and suicide attempts). Dose-response patterns for suicide attempts were observed in all types of victims. Moreover, the respondents who experienced frequent childhood emotional neglect were 14 times more likely to have attempted suicide. Conclusion: Childhood maltreatment was associated with mental health in adulthood. The findings show the need for early detection and intervention of victims of childhood maltreatment to minimize its negative impact on adult mental health. Article The Experiences of Health Care Workers during the COVID-19 Pandemic in Korea: a Qualitative Study Background: During the coronavirus disease 2019 (COVID-19) pandemic, health care workers (HCWs) have faced multiple physical and psychological challenges while carrying out their duties. In this study, we examined the experiences of specific groups of HCWs during the pandemic. Methods: From 18 November to 30 December 2020, we conducted a qualitative study using semi-structured, face-to-face interviews with four groups of 14 HCWs in three cities (Seoul, Daegu, and Gwangju) in South Korea. The HCWs who participated in the focus groups included physicians, nurses, medical practitioners, and cleaning staff who directly or indirectly cared for patients during the COVID-19 epidemic. Interviews were transcribed verbatim and analyzed using the consensual qualitative research approach. Results: Our qualitative data analysis revealed four main domains: work-related struggles, personal life-related struggles, psychological stress, and health-related struggles. Health care providers were challenged by working in critical situations and were overwhelmed by heavy workloads, fear of infection, lifestyle changes, and psychological and physical struggles. Conclusion: Our findings could serve as a foundation for establishing health care systems and policies that help HCWs cope with occupational stress, thus increasing their ability to adapt to the ongoing COVID-19 pandemic. Article Trends in the Prevalence of Major Depressive Disorder by Sociodemographic Factors in Korea: Results from Nationwide General Population Surveys in 2001, 2006, and 2011 Background: This study investigated trends in the prevalence of major depressive disorder (MDD) by sociodemographic factors in South Korea. Methods: National samples of the general population aged 18 years or older collected from the nationwide Korean Epidemiologic Catchment Area surveys conducted in 2001 (n = 6,206), 2006 (n = 6,466), and 2011 (n = 5,986) were used. For MDD diagnosis, we conducted face-to-face interviews using the Korean version of the Composite International Diagnostic Interview. We performed logistic regression analyses stratified by gender, after adjusting for other sociodemographic variables, to calculate the 2006-to-2001 odds ratio (OR) and 2011 to-2001 OR by subgroups of sociodemographic factors to explore the association of MDD prevalence with sociodemographic factors over time. Results: The prevalence of MDD in the general population of South Korea increased steadily from 2001, to 2006, and to 2011 (1.6%, 2.5%, and 3.1%, respectively). Among the men, the prevalence of MDD continued to increase significantly in 18-29 years of age group (2006: adjusted OR [AOR], 3.32; 2011: AOR, 7.42), at-risk drinking group (2006: AOR, 3.56; 2011: AOR, 4.77), and not living with a partner group (2006: AOR, 3.24; 2011: AOR, 3.25). Meanwhile, among the women, the prevalence of MDD continued to significantly increase in the below-average household income group (2006: AOR, 2.58; 2011: AOR, 2.59), at-risk drinking group (2006: AOR, 2.02; 2011: AOR, 2.47), and unemployed group (2006: AOR, 1.48; 2011: AOR, 2.04). Conclusion: This study may provide significant information for public policymakers to allocate sufficient health resources on MDD to vulnerable groups, particularly, men aged 18-29 years and women living in households with below-average income, and for clinicians to develop appropriate screening and treatment modalities for MDD. Article Black hole attack detection in vehicular ad-hoc network using secure AODV routing algorithm Vehicular ad hoc networks (VANETs) has received significant attention in the research domain of intelligent transportation system (ITS) as they provide safety and security to drivers and passengers. As compared to mobile ad hoc networks (MANETs), VANETs are mainly different in terms of characteristics, and system architecture. Security in VANET has been an important issue as it effects the communication between both (V-2-V) vehicles-to vehicle and (V-2-I) vehicle to infrastructure. In VANET, malicious attacks affect the security of the networks, and it is necessary to identify and prevent such security attacks. In VANET network, any node can function as a router for the others nodes, and a malicious node connected to the network may inject spoofed routing tables to the other nodes thereby affecting the operation of the network. To overcome this issue, a secure AODV routing protocol is developed for detection of black hole attack in this paper. The proposed method is a modified version of the original AODV routing protocol with improvements in the RREQ packet and RREP packet protocols. For added security, a cryptography function-based encryption and decryption is included to verify the source and destination nodes. The proposed approach is demonstrated on a NS-2.33 simulator using different network parameters like drop packets, end-to-end delay, and packet delivery ratio (PDR) and routing request overhead. Results demonstrate that the proposed method outperforms existing AODV routing protocol under black hole attack and improves the network performance. Article Blockchain for public health care in smart society Blockchain is a new technology that demands more efficient and scalable techniques to incorporate it with business models. Therefore, in this paper, we propose a blockchain-based smart healthcare business model, which keeps customers at the center of business. Our proposed smart healthcare business model can predict customer's status and is able to give rewards according to the business rules set by participating organizations. However, businesses also demand something in return, and in our scenario, we are concerned about data in the wild. Our model fetches "data in the wild" from the Internet of Medical Things. Nevertheless, this model can be applied to any business scenario where a customer reward system exists. Our proposed model focuses more on customers and business while utilizing technology to ease the customer and other parties involved in the business. This fusion makes business more effective as the organization can determine the path of business and make decisions accordingly. Article Directionally Patterned Large-Area Poly(3-hexylthiophene) Field-Effect Transistors via Flow-Blade Printing Method Using Coffee-Ring Effect: Uniform Performance Regardless of Pattern Fabrication Condition and Applications To apply the solution-processed organic field-effect transistors (OFETs) to practical device manufacturing, there is a need for a processing technology capable of patterning organic semiconductors with high reproducibility and uniform shape in a large area. Herein, the facile, scalable strategy to fabricate poly(3-hexylthiopehene-2,5-diyl) (P3HT) patterns through the flow-blade coating process. Controlling the stick-slip motion by the coffee-ring effect in low-/high-speed conditions induces the repetition of the advancing and receding of the contact line of the solution, which enables us to fabricate deposition of P3HT in low-speed regions and spacing in high-speed regions. Various surface-modified dielectrics with self-assembled monolayers (SAMs) influenced the morphological properties of patterned P3HT and indicated that the optimum condition for OFETs was achieved in alkyl-terminated SAM-treated cases. Additionally, various analysis tools including grazing-incidence wide-angle X-ray diffraction and near-edge X-ray absorption fine structure reveals that the flow-blade coating produces P3HT patterns with enhanced edge-on orientation compared to spin-casting and forms electrically and morphologically consistent films regardless of the pattern width. As a result, the practical large-area integrated OFET arrays on 4 in. wafer substrate exhibited highly uniform electrical characteristics (average field-effect mobility of 0.095 cm(2)/(V s)) with a deviation value of 0.0109. Article Analgesic efficacy of pre-emptive local wound infiltration plus laparoscopic-assisted transversus abdominis plane block versus wound infiltration in patients undergoing laparoscopic colorectal resection: results from a randomized, multicenter, single-blind, non-inferiority trial Background Transversus abdominis plane (TAP) block is considered a reliable locoregional technique for pain control after laparoscopic colorectal surgery. However, no clear benefit of TAP block over wound infiltration has been demonstrated by the current literature. This multicenter randomized clinical trial tested the non-inferiority of wound infiltration (WI) compared to WI plus laparoscopic-assisted TAP block (L-TAP). Methods All patients with colorectal cancer and diverticular disease scheduled for laparoscopic resection at the Division of General and Hepatobiliary Surgery, University of Verona Hospital Trust, Verona, Italy and at the Colorectal Cancer Center, Kyungpook National University Medical Center, Kyungpook National University, Daegu, Korea, between April 2018 and March 2019 were considered for the trial. Patients were randomly allocated to either the WI group or the WI plus L-TAP group in a 1:1 allocation ratio. In total, 108 patients entered the study and 102 patients were analyzed; 50 patients received WI plus L-TAP and 52 patients received WI. The primary end point was the efficacy in pain control at 6 h measured according to Numeric Rating Scale (NRS). Secondary aims evaluated pain control at 12, 24, 48 and 72 h and other short-term results related to pain management. Results Estimation of pain intensity at 6 h was comparable between the two groups (p = 0.16) with a mean (95% CI) difference in pain scores of 0.94 (- 0.13 to 2.02). No differences in pain scores were observed at other interval times or considering analgesic consumption, return of bowel function, postoperative complications and length of hospital stay. Conclusion This study suggests that adding TAP block to WI does not affect pain control, amount of analgesics and other short-term outcomes. Article Comparative analysis of genetic characterization of β-lactam-resistant Escherichia coli from bulk tank milk in Korea Background: This study was conducted to analyze the genetic characteristics of 41 beta-lactam-resistant Escherichia coli isolates, which are one of the common causes of environmental mastitis, isolated from the bulk tank milk of 290 dairy farms in five factories operated by three dairy companies in Korea. Results: Analysis of the phenotypic and genotypic characteristics of beta-lactam-resistant E. coli isolates revealed differences between factories even within the same company. Isolates from factory A1 and C1 showed high resistance to cephalothin (76.9 and 100%, respectively), which is a first-generation cephalosporins, whereas resistance to tetracycline was showed by only the isolates from factories B1 (60.0%), C2 (66.7%), and C3 (100%). Although all the 41 beta-lactam-resistant E. coli isolates were positive for bla(OXA-1), bla(TEM- 1) was highly prevalent in isolates from factories C2 (100%) and C3 (100%). Among 17 isolates resistant to both beta-lactams and aminoglycosides, the most common multilocus sequence type was ST399 (13isolates, 76.5%). Furthermore, 2 (11.8%) and 12 (70.6%) isolates belonged to the phylogenetic groups B2 and D, respectively, which are invasive strains that cause intestinal infections, respectively. The predominant serogroup was O15 (70.6%), which is a globally distributed extraintestinal pathogen. Interestingly, one isolate from factory A1 belonged to O157 and carried six virulence genes, simultaneously. Conclusions: Although E. coli isolates were isolated from bulk tank milk, and not the clinical mastitis samples, the presence of the phylogenetic groups B2 and D, and the serogroups O15 and O157, which harbor antimicrobial resistance genes and virulence factors, can pose a threat to public health. Article Comparison of different types of covered self-expandable metal stents for malignant colorectal obstruction Background Stent migration is one of the main drawbacks of covered self-expandable metal stent (SEMSs), occurring in up to 40% of malignant colorectal obstruction management cases. Various types of covered SEMSs have been developed to reduce this risk. We aimed to compare the effectiveness and complication rates of the flare-type covered SEMS (Flare) with those of the double-layered covered SEMS (ComVi). Methods We performed a prospective, randomized study in four tertiary referral centers between July 2016 and April 2018. Patients with malignant colorectal obstruction were eligible for the study. The primary outcome was migration rate as observed within the first month. Rates of technical success, clinical success, and complications within the first month were also assessed. Results A total of 60 patients were included (mean age, 70.5 +/- 12.5 years; male, 31 [51.7%]). Flare and ComVi stents were applied in 30 patients each. The Flare and ComVi groups showed comparable technical success rates (90% [27/30]vs.96.7% [29/30],p = 0.605) and clinical success rates (85.2% [23/27]vs.75.9% [22/29],p = 0.589). Migration occurred in three (11.1%) and four (13.8%) cases in the Flare and ComVi groups, respectively, without significant difference (p = 0.99), and the risk of other complications, including perforation and re-obstruction, did not differ between the two groups. Conclusions Our study indicates that both flare-shape and double-layered covered SEMSs are equally effective options for the management of malignant colorectal obstruction with low migration rates when compared with previously reported migration risk of covered SEMS. Article Comparison of surgical outcomes among different methods of esophagojejunostomy in laparoscopic total gastrectomy for clinical stage I proximal gastric cancer: results of a single-arm multicenter phase II clinical trial in Korea, KLASS 03 Background Laparoscopic distal gastrectomy for early gastric cancer has been widely accepted, but laparoscopic total gastrectomy has still not gained popularity because of technical difficulty and unsolved safety issue. We conducted a single-arm multicenter phase II clinical trial to evaluate the safety and the feasibility of laparoscopic total gastrectomy for clinical stage I proximal gastric cancer in terms of postoperative morbidity and mortality in Korea. The secondary endpoint of this trial was comparison of surgical outcomes among the groups that received different methods of esophagojejunostomy (EJ). Methods The 160 patients of the full analysis set group were divided into three groups according to the method of EJ, the extracorporeal circular stapling group (EC;n = 45), the intracorporeal circular stapling group (IC;n = 64), and the intracorporeal linear stapling group (IL;n = 51). The clinicopathologic characteristics and the surgical outcomes were compared among these three groups. Results There were no significant differences in the early complication rates among the three groups (26.7% vs. 18.8% vs. 17.6%, EC vs. IC vs. IL;p = 0.516). The length of mini-laparotomy incision was significantly longer in the EC group than in the IC or IL group. The anastomosis time was significantly shorter in the EC group than in the IL group. The time to first flatus was significantly shorter in the IL group than in the EC group. The long-term complication rate was not significantly different among the three groups (4.4% vs. 12.7% vs. 7.8%; EC vs. IC vs. IL;p = 0.359), however, the long-term incidence of EJ stenosis in IC group (10.9%) was significantly higher than in EC (0%) and IL (2.0%) groups (p = 0.020). Conclusions The extracorporeal circular stapling and the intracorporeal linear stapling were safe and feasible in laparoscopic total gastrectomy, however, intracorporeal circular stapling increased EJ stenosis. Article Development of Multi-Quadrotor Simulator Based on Real-Time Hypervisor Systems Today, simulator technology has been widely used as an important part of quadrotor development such as validation and testing. A good quadrotor simulator can simulate the quadrotor system as closely as possible to the real one. Therefore, in case of multi-quadrotor simulator, the simulator should not only can simulate a multi-quadrotor system, but also every quadrotor should be able to leverage their own resources. To solve this issues, in this paper, we present a hypervisor-based multi-quadrotor simulator. We used RT-Xen as hypervisor, a real-time Xen hypervisor. To ensure every quadrotor runs in real-time manner, we implemented quadrotor simulator in Litmus-RT which is a real-time extension of Linux. In this paper, we conducted some testing and performance evaluation for particular cases on our multi-quadrotor simulator: step-input responses, computation time, and response times. Based on the performance evaluation, our hypervisor-based multi-quadrotor simulator environment is proven to meet the real-time requirements. The results show that three important tasks in quadrotor system: Stability Controllability Augmented System (SCAS), Equation of Motion (EOM), and waypoint following task, are finished before their deadlines; in fact, 20 ms, 10 ms, and 40 ms before the deadlines for SCAS, EOM, and waypoint following, respectively. Article Incidence and Prevalence of Juvenile Systemic Lupus Erythematosus in Korea: Data From the 2017 National Health Claims Database Objective. The purpose of the present study was to investigate the prevalence and incidence of juvenile systemic lupus erythematosus (JSLE) in Korea. Methods. The data were collected from the National Health Insurance Claims Database of Korea. JSLE was identified using the diagnostic code M32 from the Korean Standard Classification of Diseases. Patients between 5 and 18 years old, who had at least 1 claim for JSLE from January 1, 2016, to December 31, 2017, as final diagnosis, were analyzed in the study. For prevalent cases, patients who used, at least 1 time, any type of medical services with a diagnostic code of M32 were selected. For incident cases, patients who did not use medical services with the M32 code 1 year prior and who were newly registered in 2017 were defined. Change-point analysis was used to find the age at which changes in prevalence and incidence occurred. Results. The prevalence of JSLE was 6.92 per 100,000 persons and the incidence of JSLE was 2.76 per 100,000 person-years in patients between 5 and 18 years old. The prevalence and incidence of JSLE were higher in females than in males. According to the change-point analysis, we found that the incidence and prevalence of female patients increased rapidly at the ages of 14 and 15 years, respectively. Conclusion. This Korean population-based epidemiological study of JSLE showed similar epidemiologic profiles to Asian population in other studies. The distribution of age, ethnicity, and pubertal status are important factors that influence population estimates of JSLE incidence and prevalence. Review A short review: Comparisons of high-throughput phenotyping methods for detecting drought tolerance Drought is a major threat worldwide for crop production, especially due to the rapid climate changes. Current drought solutions involve improving irrigation system, rainwater harvesting, damming, cloud seeding, and changes of cultivation methods. Despite effective, each solution has economic, environmental, and temporal drawbacks. Among all solutions, the most effective, inexpensive and manageable method is the use of drought-tolerant cultivars via plant breeding. However, conventional plant breeding is a time-consuming and laborious task, especially for phenotypic data acquisition of target traits of numerous progenies. High-throughput phenotyping (HTP) is a recently developed method and has potential to overcome the mentioned issues. HTP offers massive, accurate, rapid, and automatic data acquisition in the breeding procedure and can be a breakthrough for developing drought resistant/tolerant cultivars. This study introduces various methods of HTP to detect drought stress, which can accelerate the breeding processes of drought-tolerant cultivars to provide helpful guidelines for breeders and researchers to choose appropriate methods. Article Oral Administration of Germinated, Pigmented, Giant Embryo Rice (Oryza sativa L. cv. Keunnunjami) Extract Improves the Lipid and Glucose Metabolisms in High-Fat Diet-Fed Mice Obesity is a significant risk factor for chronic diseases. The effect of ethanol extract from germinated Keunnunjami, blackish-purple rice with a giant embryo, compare to ordinary brown rice, on the body weight and lipid and glucose metabolism in high-fat diet-fed mice was analyzed. Mice were fed with a high-fat diet-fed for 3 weeks and then orally administered with either distilled water (HF) or extract (0.25%, w/w) from brown, germinated brown, Keunnunjami, and germinated Keunnunjami rice for 4 weeks. Control mice were fed with a normal diet and orally administered with distilled water. The HF group showed markedly higher body weight and triglyceride, cholesterol, fatty acid, glucose, and insulin levels than the control group. However, the oral administration of rice extracts ameliorated this high-fat diet-induced obesity, hyperlipidemia, and hypoglycemia through the modulation of adipokine production, lipogenic and glucose-regulating enzyme activities, and mRNA expression of genes associated with lipid and glucose metabolism. The germinated Keunnunjami extract exhibited greater hypolipidemic, hypoglycemic, and body weight-lowering effects than the other rice extracts. The results demonstrated that germination could further enhance the physiological properties of rice and that germinated Keunnunjami extract has a strong therapeutic potential against high-fat diet-induced obesity, hyperlipidemia, and hyperglycemia.
